So Confused.... RIP Carpenters Wrasse

So I come home today to find my beloved carpenters wrasse dried out on the floor. What confuses me is how it got out? I have a screen lid covering the tank and it was in the same spot as when I left. Such a shame.

That stinks...any chance you can take a picture of the screen top, maybe we can help prevent another jumper.
 
I don't have a close up but will post one tonight. I was baffled by this. Best part was that my cat, which seems to find just about every insect in the house, did not find the fish and eat it as I would have thought it to be dead in the tank.

Hard to tell exactly from the photo but it looks like there is some room to jump out by the back. I run my cords through the screen (you have to cut a tiny slit to get the power cord through) to prevent having any openings. I run the cords near the corner by the overflow for two reasons: fish can't get a straight shot at the opening and I can swing my screen off the tank like a swivel for cleaning. So far my screen is winning though I have heard several times my fish bouncing off the screen.
